Responsibilities
- Set up and break down program operations
- Set up and break down program operations
- Conduct intake and assessments for new participants
- Develop service plan with a focus on financial stability and re-housing
- Provide individualized resources and referrals to community programs
- Assist clients in advocating for services for which they qualify
- Provide employment development support
- Work on a re-housing plan with client
- Develop partnerships with other community based organizations
- Outcomes and data management into HMIS for participant activities and accomplishments
- Follow-up with participants who have exited the program
- Assist in oversight of interns during program operations
- Meet for supervision bi-weekly
- Other duties as assigned
